When you arrive to your first visit with us, you will be asked to provide the following information (please note — Depending on your insurance type or the condition we are treating, there may be additional forms provided at the clinic):

• Welcome Page (PDF)/(DOCX)
• New Patient Forms (PDF)/(DOCX)
• Patient History Form (PDF)/(DOCX)
• Physician’s prescription/referral for Physical or Hand Therapy
• Insurance Card
• Photo ID

Insurances We Accept

We are contracted and participate with most local and national insurance plans. If you don’t see your insurance listed below or for questions regarding which plans we accept, please contact our office nearest you.

Don’t let the fear of billing and insurance stand in the way of getting the treatment that you deserve. We offer cash pay rates for uninsured or out-of-network patients as well as flexible payment plan options. Call us to inquire about further details!
